This article will explain how to submit a related services referral using the updated process.
New for 24-25 SY: If your school is working with LiveOT, you will not need to specify this during the referral process. Referral tickets with virtual occupational therapy selected for LiveOT schools will automatically be routed to the LiveOT supervisor for LiveOT assignment.
If a student is newly enrolled, has just had an IEP developed, or has had a new related service added to the IEP, a related services referral form must be submitted. Please follow the steps below to submit the form.
Based on feedback from schools, the updated process requires a separate form to be submitted for each related service area. The allows for better tracking methods and also makes it easier to identify the vendor and therapist assigned for each service area.
Follow the steps below:
- Open the Related Services Referral Request form and enter relevant information such as your email address, student's ID number, and school name.
- If applicable, enter the priority and zip code (for face to face/in person requests).
- For Type of Request, select "Therapy" and then enter the preferred vendor and/or therapist (if applicable).
- Enter the Related Services Therapy Type based on the options in the dropdown menu. Additional fields will appear depending on the area selected. In the screenshot below, "Counseling" was selected, so fields for the Counseling setting and Counseling minutes per month are displayed.
- Lastly, enter the ticket name according to suggested naming conventions and include any relevant details in the ticket description. The click "Submit."
- The person who submitted the form will receive an automatic email notification once the ticket has been received.
