How should I log a therapy session?

This article will explain how to create a log entry for a therapy session for Tier IV Providers and properly document this within the Pearson Online Classroom.

Listed below are the components used to create a log entry to document the provision of 
related services.

  • About: Student
  • System: Student
  • Location: School Location (Default)
  • Contact Date: Date and Time of Contact
  • Contact Type: Select the appropriate contact type used to communicate with the 
    student/caretaker and or deliver services. All successful and attempted contacts and 
    service delivery must be logged. Ensure that you communicate with the school to 
    determine preferred contact types for logging.
    • Phone Call- Successful
    • Phone Call- Left Message
    • Face-to-Face
    • Face-to-Face- Did Not Attend
    • LiveLesson- Individual
    • LiveLesson- Did Not Attend
    • LiveLesson- Group
  • Areas and Categories: Related Services
    • Select the specific option that fits the type of therapy provided and duration (example: select "Physical Therapy - 30 Minutes" for a session of PT that was scheduled for 30 minutes)
    • If you don't see your specific type of related service, select "Other Related Service" with the appropriate duration
    • If the session lasted longer than the options provided, you may select more than one, for example, selecting "Social Work - 15 minutes" and "Social Work - 60 minutes" for a session that lasted 75 minutes
  • Contactees: Student, Learning Coach, and/or Caretaker
  • Comments: Use this area to document the provision of related services. Details should 
    include the duration, service type, and goals addressed.

Common Mistake #1

Mistake: Selecting the "Speech Therapy" Area and Category and then selecting a therapy check box underneath such as "Individual Therapy" check box.

How to Fix: Select the "Related Services" Area and Category and then select "Other Related Services" check box with the appropriate time of the session.

NOTE: If your related service area is listed (OT, PT, Counseling, or Social Work) please choose that one and only use "Other Related Service" if it isn't not in the options list.

Common Mistake #2

Mistake: Selecting "Occupational Therapy" system.  This should be used for LiveOT ONLY. How to Fix: Select "Student" system instead.  Then select Special Education > Related Services for the area and category.

Reminders

Your log entry doesn't need to be extremely detailed if you are documenting details related to the student's participation as well as any data collected on IEP goals in another location, but at a minimum the comment of the log entry should include the duration of the session, the type of service provided, and the goals/topics addressed.

  • Example of a simple log entry comment for therapists tracking data in another location:
    •  "I met with Brady from 10:30 AM to 11:00 AM for a virtual counseling session.  We worked on coping strategies related to his IEP goals."
  • Example of a comment from a more detailed log entry if the therapist is not tracking data in another location:
    • "Mrs. Demo of Demo Therapy LLC provided 30 minutes of Occupational Therapy. The student arrived on time and was attentive and participatory throughout the entire session. Mrs. Demo and the student worked on the following goal: By 6/20/25, during therapy handwriting activities, John will improve legibility..... with 80% accuracy from a current baseline of 65% accuracy in 7/10 trials as measured by therapy data collection. Today, John and Mrs. Demo focused on the lowercase forms of j and q. John was able to write the lowercase form of j correctly on 3/3 trials with 100% accuracy, fully bringing j underneath the writing line. However, he struggled with writing correctly. John continues to write the lowercase form of q similar to that of a lowercase a, failing to bring q underneath the writing line on ⅔ trials. John was given a homework practice assignment to work on this skill. Mrs. Demo will work on the letter q in the upcoming sessions."
