How do I create a log note for a meeting?

This article will explain how to create a log note for participation in meetings related to the IEP process. This can include annual reviews, amendment, evaluation, or eligibility meetings.

All communication must be logged in the Pearson Online Classroom, including participation in meetings.  Use this log entry type to document participation in an IEP meeting for students in your section. Schools typically have a designated notetaker for each meeting.  This is often the student's case manager or the LEA designee for the meeting.  The notetaker will create a detailed log entry to capture discussion at the meeting, but participants should still log their attendance.

If you are an evaluator for the student and do not have them within one of your sections, you will not be able to create a log entry for the student if you have Tier IV access (those will Tier V access will be able to search for the student and create a log note).  Use the guide below to create a log note for IEP and eligibility meetings:

  • Contact Type: Phone Call – Successful, Phone Call – Unsuccessful, Phone Call – Left

  • Areas and Categories: Special Education > IEP Meeting, or Eligibility/Evaluation Meeting
  • Sample Comments: I shared Karson’s progress with the parents and team. Karson 
    actively participates in all therapy sessions and has mastered his current typing goal. I 
    proposed a new goal to continue to address Karson’s fine motor needs. I continued to 
    recommend 60 minutes/month of virtual occupational therapy services. Karson’s parents agreed with this recommendation and shared that they are pleased with his progress.

Here is a sample entry for the eligibility meeting: 

  • I attended a 45 minute eligibility meeting for Jordyn and reviewed the results of my evaluation report with the parents and team.  Team agreed that Jordyn continues to qualify for services as a student with OHI (please see report for more details).  At the conclusion of the eligibility discussion, parents and team agreed to excuse evaluators from the meeting and moved into student's annual review.
