This article explains what to do if a therapist or evaluator resigns.

If a therapist or evaluator resigns, it is important to notify the Operations and Related Services team as soon as possible to ensure that the caseload is reassigned in a timely manner.  This reduces the likelihood of the student missing services which could require compensatory minutes to be made up, or for an evaluation to become overdue and therefore out of compliance.

When you are aware of a provide resignation, the vendor main contact should complete the Therapist/Evaluator Resignation Form.  You will be prompted to include the following information about the provider:

  • Provider's name
  • Total number of students on caseload
  • Total minutes of students on caseload
  • Number of incomplete evaluators
  • Last day worked
  • Last day Pearson Online Classroom access is needed

If the therapist has less than 20 students on their caseload, you will be prompted to enter the ID number and minutes per month (according to IEP) for each student on the caseload.  If the therapist has 20 or more students on their caseload, you will be prompted to upload a file that lists all student IDs and minutes for each student.  This information is requested to ensure that no students are missed when transferring them to another therapist.

Remember that all log entries must be completed before the therapist's last day.

Submitting the resignation form will notify the Vendor Manager and generate a new referral ticket so that the therapy referrals and/or evaluation referrals can be reassigned.
